    Check the three gates

    Verify the actor's permission for the exact action, match the approval tier against the real (not claimed) blast radius, and confirm audit evidence hooks exist. Quote each check.

  2. Step 2

    Reject specifically

    Reject the plan quoting each gap: wrong tier, missing evidence hooks, unstaged rollout for the real radius. The rejection teaches; it does not merely block.

  3. Step 3

    Pass the revision

    Review the revised plan (staged rollout, correct approvers, evidence hooks), pass all three gates, and record the decision with approvers and evidence named.
